- Psychosocial Support
- Providing emotional and social support to patients and families
- Helping patients cope with illness, hospitalization, disability, or crisis situations
- Patient Counseling and Guidance
- Counseling patients and caregivers about treatment-related concerns
- Supporting decision-making and adjustment to health conditions
- Assessment of Social and Economic Needs
- Identifying financial, family, housing, transportation, or social barriers affecting care
- Linking vulnerable patients to available support services
- Referral and Coordination of Services
- Connecting patients with community resources, support programs, insurance services, or social assistance organizations
- Coordinating with government and non-government support systems
- Discharge Planning and Continuity of Care
- Assisting in preparing patients for discharge from the hospital
- Protection of Vulnerable Groups
- Supporting children, elderly patients, survivors of violence, people with disabilities, and other vulnerable populations
- Assisting with safeguarding and protection concerns
- Health Education and Advocacy
- Educating patients and families about their rights and available services
